Dear Patients:

 

It is with mixed emotions that I am announcing I will no longer be consulting with patients in the office, effective (7/5/2023). I will only be seeing patients in the hospital. It has been a great pleasure providing for your health care needs over the years, and it is not easy for me to give it up.

 

As of (07/05/2023), My partners Dr. Turuvekere Jayaram and Dr. Salman Alim are available in the office to continue your pulmonary needs. I am pleased that you have the opportunity to have one of them as your physician. If you have a upcoming appointment after this date and decide to continue with the practice a medical assistant will call you to move it or make any changes necessary.

 

Your medical records are confidential, and a copy can be transferred to another doctor or released to you or another person you designate only with your permission. If you plan to continue with this office, you can sign an authorization form to release your files to the doctor that you choose. If you choose to see a different physician, please sign a authorization form and return it to my office as soon as possible so we may transfer your records to your new doctor. Until then, your records will remain on file at my former office.
